
Clinical Pharmacist

- Bachelor's degree in pharmacy
- Active Pharmacist licensure
- Proficiency in prescription formulas and dosage calculations
- Working knowledge of Ohio Board of Pharmacy regulations
- 1-2 years of experience in pharmacy practice
- Foster a professional, collaborative, and patient-focused environment, maintaining open communication with pharmacy leadership, physicians, nurses, pharmacists, and technicians.
- Conduct thorough patient assessments and provide pharmaceutical recommendations to maximize therapeutic effectiveness, minimize adverse events, and promote cost-effective medication use.
- Educate and counsel patients on medication therapies, offering timely, clear, and concise responses.
- Ensure adherence to the seven patient “rights”: right patient, right dose, right route, right time, right drug, right information, and right documentation.
- Report medication errors and assist in risk assessment and mitigation strategies.
- Maintain compliance with regulatory requirements and organizational policies.
- Support protocol development, implementation, and optimization to enhance medication safety.
- Lead projects aimed at improving patient care quality and promoting cost-effective medication use.
- Oversee drug distribution processes, including verification, order entry, clarification, technician supervision, and timely delivery.
- Manage drug inventory by adjusting par levels, preventing shortages, and minimizing waste.
- Collaborate with Pharmacy Buyers to ensure appropriate stock levels and supply-demand alignment.
- Identify opportunities for pharmacy process improvement and implement effective solutions.
- Take initiative in ongoing professional development, engage in clinical education, and participate in relevant committees.
- Assist with the orientation and training of new hires and serve as a preceptor for pharmacy students and residents.
