- Provide comprehensive case management, beginning with performing an initial assessment of the needs and goals of the client, making appropriate referrals, and tracking progress towards goals over time.
- Assisting older ORR-eligible populations with access to mainstream aging services in the community, through activities such as providing information about, or improving access to information, about supportive services, nutrition services, meal delivery, prevention of elder abuse, senior community centers, and intergenerational activities.
- Detail each client interaction in accordance with program requirements.
- Input client information in database(s) and assist with tracking and reporting as needed.
- Build and maintain relationships with area service providers for the benefit of clients.
- Create opportunities for older ORR eligible populations to live independently and connect with their communities to avoid isolation.
- Assist older refugees with application for public benefits such as social security income, public housing, Medicaid, snap and cash assistance benefits
- Assist older ORR- eligible populations on the path to citizenship by providing referrals to civics and English classes, as well as immigration referrals.
- Use personal, insured vehicle and/or public transportation (where available) to travel and transport clients and materials as needed throughout the service delivery area.
- Train and lead volunteers.
- Perform other duties as assigned.
- High school diploma required; bachelor's degree in social work or related field of study preferred.
- A minimum of 1 year of case management or human field experience is strongly preferred.
- Experience working with refugee and/or immigrant populations preferred.
- Strong relationship building, diplomacy, and networking skills; ability to effectively build internal and external relationships.
- Demonstrated success working and communicating effectively in a multi-cultural environment.
- Self-starter with excellent problem-solving skills combined with the proven ability to multi-task, prioritize duties, and manage time effectively.
- Attention to detail and accuracy in work product.
- Fluent in English, both spoken and written; proficiency in language(s) spoken by client group is desired (Spanish, French, Pashto, Dari, Arabic, Swahili, and others).
- Proficient in Microsoft Office applications (Word, Excel, Outlook); ability to use the internet and other digital tools for data entry, research, and problem-solving purposes.
- Valid driver's license and access to a personal, insured vehicle.
