Job DescriptionJoin UChicago Medicine, as an Ambulatory Medication Access Coordinator on the Meds2Beds transitions of care team in the Pharmacy Department. In this role, you will be responsible for various tasks related to prescription benefits and medication access for patients discharging home from the hospital. You will gain exposure to inpatient care, outpatient pharmacy, and transitions of care. The Ambulatory Medication Access Coordinator plays a key role on our Meds2Beds team in order to provide optimal patient care at the time of discharge. UChicago Medicine is dedicated to providing patients with the personal service necessary to ensure that they achieve the most benefit from their medication therapy.In this role, the Ambulatory Medication Access Coordinator (AMAC) performs necessary functions to ensure medication access for patients discharging home from the hospital. Other areas of responsibility include pre-screening patients for pharmacy prescription insurance, enrollment of patients into the Meds2Beds service, managing cost and payment associated with take-home medications, identifying opportunities for pharmacist intervention on prescriptions, completion of necessary paperwork for prior authorizations, performing prescription insurance test claims via consultation from providers, and communication with the multidisciplinary care team.Essential Job Functions:
Facilitate enrollment of patients into the Meds2Beds transitions of care discharge medication service
Conduct patient prescription insurance investigation and insurance input into outpatient pharmacy system
Process and adjudicate medication claims through insurance
Respond to general pages from providers and nurses regarding enrolled patients
Respond to medication consult pages from inpatient provider or team member
Communicate with patients, nurses, physicians, pharmacists, social workers, case managers, etc. to coordinate discharge medications for patients
Document appropriate notes in patient charts within the electronic medical record
Coordinate payment for copays or medications not covered by insurance with the patient and/or Care Coordination
Fill prescriptions in the outpatient pharmacy
Dispense medications in the outpatient pharmacy
Transport medications from the outpatient pharmacy to patient bedside, nursing unit, or discharge lounge depending on patient status and inpatient location
Complete prior authorization forms for select medications
Provide exemplary customer service to all patients
Perform other duties as requested by management
Required Qualifications:
Associate or Bachelor’s degree
Current (or eligible for) Illinois Pharmacy Technician Licensure
3 to 5 years of pharmacy technician experience in a retail pharmacy (exceptional aptitude or knowledge may be considered in lieu of experience)
Knowledge of and ability to use computers and a variety of computer programs including word processing, database management, and spreadsheets
Strong verbal and written communications skills
Ability to effectively manage conflicts and resolve problems
Effective interpersonal skills, including the ability to positively influence collaborative alliances, promote teamwork, and ensure a high degree of internal and external customer satisfaction
Ability to function effectively in a fast-paced and changing environment with multiple priorities
Knowledge of and ability to perform insurance carrier/managed care review
Preferred Qualifications:
Knowledge of medication reimbursement
Knowledge of healthcare/prescription insurance billing, prior authorization, and coding

Shift: Rotating 8.5 hour shift; includes a weekend staffing component (you may work Saturday or Sunday, both days, or neither day on any given weekend)

In this role, we anticipate that you will be scheduled to work a rotating shift. Shifts are typically 8.5 hours with shift start times ranging from 7am to 11am and lasting for 8.5 hours. We anticipate that you will be scheduled to work 100% at the Hyde Park location.
