Licensed Vocational Nurse (LVN)

• Deliver patient-centered care in the home setting, conducting nursing visits as prescribed by physicians.
• Monitor and manage patients’ health conditions, supporting recovery and improving quality of life.
• Maintain accurate and comprehensive patient records, ensuring detailed documentation of visits, care provided, and patient responses.
• Collaborate with other healthcare professionals, including RNs, physicians, and specialists, to optimize patient care plans and achieve care goals.
• Educate patients and families on managing chronic conditions, medications, and lifestyle choices to support health outcomes.

• Licensure:

• Current Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license in the state of practice.

• Experience:

• 1 year of clinical experience and 1 year of home health experience.
• Education:

• Graduate of an accredited practical or vocational nursing program.
• Certifications:

• CPR/BLS certification (required, must be maintained throughout employment).
