- Assist the Occupational Therapist in implementing patient-specific treatment plans, focusing on improving daily living skills, fine and gross motor skills, cognitive function, and overall independence.
- Provide therapeutic interventions as prescribed by the OT, including exercises, activities, and adaptive techniques.
- Educate patients and caregivers on home exercise programs, adaptive equipment, and strategies for managing daily tasks more effectively and safely.
- Monitor patient progress, document treatment sessions, and report any changes in condition or progress to the supervising OT.
- Assist patients with tasks such as dressing, grooming, bathing, meal preparation, and home safety assessments.
- Ensure patient safety during therapy sessions and encourage maximum participation and independence.
- Provide guidance on environmental modifications, assistive devices, and home safety to help improve the patient's quality of life and functionality.
- Maintain accurate, timely, and complete documentation of patient care activities in compliance with agency policies, state regulations, and insurance guidelines.
- Collaborate with the healthcare team, including nurses, social workers, and physicians, to ensure integrated and patient-centered care.
- Maintain adherence to infection control protocols and safety guidelines in the home environment.
- Associate’s degree from an accredited Occupational Therapy Assistant program.
- Valid state certification as a Certified Occupational Therapy Assistant (COTA).
- CPR and First Aid certification.
- Previous experience in home health, outpatient, or inpatient settings is preferred.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently with minimal supervision and manage time effectively in a home health environment.
- Strong clinical skills and a passion for patient care and improving functional outcomes.
- Reliable transportation and ability to travel to patient homes as needed.
- Compassionate, patient-focused, and committed to enhancing the quality of life for patients.
- Ability to lift, move, and assist patients (with assistance when needed).
- Ability to stand, walk, bend, squat, kneel, and perform other physical activities related to patient care.
