- Serve as a clinical lead within the nephrology team
- Collaborate with nephrologists, nursing, care coordinators, and dialysis providers to ensure integrated, patient-centered care
- Participate in developing and refining clinical protocols, workflows, and quality improvement initiatives
- Assist in program coordination, such as transitions of care, home dialysis support, and CKD education programs
- Enjoy a 4.5 day workweek: No weekends or evenings
- No call responsibilities

- The anticipated base compensation range for this position is $115,000 - $120,000, depending upon the applicant’s qualifications and experience. In addition to the base compensation, this position may also be eligible for a sign-on bonus and relocation bonus equaling a total target cash compensation that may be between $115,000 - $150,000.

- Graduate of an approved school for Nurse Practitioners OR Physician Assistants-Master’s Degree
- Licensed or ability to obtain licensure in the state of Illinois
- Minimum of 2 years of nephrology or internal medicine experience
