Employment Type: Full timeShift: Day ShiftDescription:​Position Summary:The Care Coordinator RN is a professional nurse who collaborates with physicians, health care team members, and patient/families to coordinate care for specific patient populations across the continuum and between care settings during an episode of illness. The Care Coordinator RN participates in the assessment, plan, monitoring, and evaluation of the ongoing care of assigned patient group as they transition through the continuum of care. He/She is responsible for coordinating the delivery of resources for effective, efficient, and appropriate patient care during an entire episode of illness. The Care Coordinator is accountable for ongoing monitoring and evaluation of patient outcomes.What the Orthopedic Case Manager will need:
  • Graduate of an accredited school of nursing. Bachelor of Science degree in Nursing required or equivalent work experience. Master’s degree in nursing (or actively pursuing master’s degree in nursing) preferred.
  • Current licensure to practice nursing in Michigan.
  • One year of experience at Mercy Health and two years of clinical experience in nursing.
  • Customer-focused interpersonal skills in order to interact in an effective manner with physicians, health team members, community agencies, and patients and families.
  • Leadership qualities including time management, verbal and written communication skills, listening skills, problem solving, decision-making, priority setting, work delegation and work organization.
What the Orthopedic Case Manager will do:
  • Appraises the need for resources necessary for care through the gathering and critical, objective evaluation of relevant data.
  • Collaborates with the health care team to manage the resources utilized for the specific client population by using clinical pathways, other care management tools and utilization of interdisciplinary case conference.
  • Compares the patient’s disease course to the established clinical pathway. Coordinates each patient’s transition along the health continuum. Facilitates communication among patient, family and health care team.
