- Long-term employment for part-time and full-time Home Health Aides.
- Those seeking full-time hours will have the option to start with part-time hours and first priority to add additional clients/shifts for up to 40 hours per week (if desired).

Compensation is based on qualifications, experience, positive employment history, and/or certifications.
QUALIFICATIONS:
- 1+ year of verifiable experience in home care or caregiving setting is required.
- Successful completion of a formal certification training program (CNA, STNA, CHHA) is preferred, and/or a written skills test and competency evaluation. HCN will train non-certified caregivers committed to building a career with our agency.
- Meet the training and/or certification requirements of the state in which the home health aide services are being provided. This includes specifications of contracted agencies for which services are being provided.
- CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
- Willingness and aptitude to perform light housekeeping in addition to patient care services as needed is required.
- Strong attendance and dependability track record with previous employers.
- Good interpersonal skills and compassion for helping others live their best lives are required.
- Demonstrated ability to read, write, and follow a written Plan of Care, incorporating attention to detail in following instructions, documenting notes, and communicating with our agency.
- Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful.
- Be at lease eighteen (18) years of age.
- Valid driver's license/ID and reliable transportation to/from client care locations is mandatory.
- Deliver direct patient care and support, following the established plan of care.
- Measure and record vital signs and other specified metrics as instructed.
- Assist with personal care tasks and activities of daily living (e.g., bathing, oral hygiene, nail & skin care, shaving, dressing).
- Perform household tasks essential to health care and living at home (e.g., meal preparation/feeding, laundry, light housekeeping).
- Assists patients in taking medications under the direction and supervision of the Registered Nurse. This includes opening a container or pill caddy & handing it to the patient, but does not include administration of the medications.
- Identify and report any abnormal findings or changes in the patient's condition.
- Help with therapy exercises under supervision of our therapists (e.g., range of motion exercises, ambulation assistance).
- Know and follow infection control policies including proper hand washing techniques.
- Report any observed or reported changes in the client's condition and/or needs to the Registered Nurse.
- Document care provided and complete all forms required for the client's records. Complete the appropriate records to document cares given and pertinent observations.
- Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager.
- Demonstrate safe practice in the use of equipment, avoiding equipment use until orientation has been provided. Notifying supervisor(s) of educational needs immediately.
- Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested and through approved channels in a very timely manner.
- Treat patients and families with respect. Maintain confidentiality in all aspects of the job.
- Accept and fulfill assignments with our agency. Exercise good judgment in accepting assignments.
- Participate in required in-service programs to meet compliance requirements. Identify learning needs to agency supervisor.
