Responsibilities include participating in the clinical management of the patient with all necessary disciplines to coordinate patient services and education across the continuum of care. Other duties as assigned.EXPERIENCE DESCRIPTION:Minimum of 3 years of acute care or dialysis experience is required. Experience relevant to Nephrology/Transplant sub-specialties is preferred.EDUCATION DESCRIPTION:Associate's degree in Nursing with a minimum of 10 years of relevant experience is required or Bachelor's degree in Nursing with a minimum of 3 years of relevant experience is required. Bachelor's degree in Nursing is preferred.SPECIAL SKILLS DESCRIPTION:Hands on patient experience in an intensive care setting. High degree of organizational skills and excellent communication skills; ability to work independently and take programmatic initiative. Knowledge of data systems, outcomes data and other clinical performance data.LICENSURE DESCRIPTION:Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact).
