- Fulfills the role of an RCC completing patient admissions.
- Serves as a resource to all other RCC's in completing admissions.
- Leads communication and coordination of care between all members of the care team.
- Assists all patients and care team members throughout the care model by acting as a patient advocate.
- Assists market manager in coordination of staffing and ensures appropriate resources are available to support patient care.
- Ensures that all new RCCs have the appropriate tools and templates to operate.
- Supports market manager in coordination of clinical and operational workflows and documents those steps.
- Assists market manager in maintaining data and reporting accuracy.
- Serves as a subject matter expert in identifying and solving negative trends and hurdles involved in health care system and incorporates solutions for system challenges.
- Builds and maintains collaborative professional working relationships with all care team members and is available to provide guidance and support during times of urgency or critical needs.
- Assists as needed in recruiting, interviewing and training of new team members.
- Performs other duties as assigned.
- Associate of Science in nursing.
- Current, active RN state licensure.
- Three to five years of acute nursing experience. Possess a general knowledge of clinical standards and outcome measurement.
- Reliable transportation and proof of liability insurance.
- Ability to support after-hours schedules and on-call needs.
- Bachelor of Science in nursing.
- Three months of experience as an acute care RN providing in-home care.
- Two to three years of recovery care coordinator experience.
