- Assist individuals with bathing, dressing, and grooming
- Help maintain personal hygiene and comfort
- Monitor prescribed medications according to the care plan
- Monitor medication schedules and document any changes
- Document assessment and align goals with the Person-Centered Treatment Plan within the EHR (Electronic Health Record) system
- Adhere to HIPAA regulations and maintain confidentiality
- Assist individuals with transferring in and out of bed or chairs
- Ensure safe movement within the home
- Prepare meals based on dietary requirements
- Encourage proper nutrition and hydration
- Regularly check vital signs (such as blood pressure, pulse, and temperature)
- Report any significant changes promptly
- Engage in conversations and provide emotional comfort
- Foster a positive and supportive environment
- Perform cardiopulmonary resuscitation (CPR), crisis intervention and medication administration using agency-trained protocols
- Maintain current state-mandated training and certifications
- Housekeeping duties, including laundry, tidying up, and organizing
- Ensure a clean and safe living space
- High school diploma or GED required
- Completion of a certified nursing assistant training program and a valid CNA license certification required
- At least one year of experience as a certified nursing assistant preferred
- Ability to adapt to various home environments
- Effective communication with individuals and their families
- Ability to work evening, weekend and holiday hours according to the team model and an on-call rotation will be required as necessary
- Non-profit human service experience highly preferred
- Demonstrated ability to work effectively as part of a team
- Ability to work in a fast-paced environment and manage competing priorities
- Demonstrated strong attention to detail
- Demonstrated strong time management and organizational skills
- Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
- Must be able to perform CPR and crisis intervention using agency-trained protocols
- Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication
- Basic experience with Microsoft Office applications, including Word and Outlook preferred
- Must possess a current, valid driver's license in state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record
