Minimum Hiring Wage: From $39.37 per hourJob Details: Scheduled hours: 36 hours per pay period. 12-hour day shift. May include some evenings and weekends, as needed.Job Description:Coordinates patient care with input from the hospice interdisciplinary team.Communicates with insurance companies for authorization andapproval for hospice services initially and ongoing through the course of care.Evaluates patients for eligibility for three levels of hospice care-respite, residential, and general inpatient care. Documents patient care to support each level of care for regulatory and payment purposes.Supervises and assigns care to hospice aides and LPNs. Prepares agenda and coordinates hospice interdisciplinary team meetings. Monitorshospice inpatient building maintenance needs and security after office hours.Collaborates, communicates, provides psychosocial supportand education to family members or significant others.
