- Collaborate with physicians, therapists and nursing team to develop and implement individualized care plans for patients.
- Coordinate care for various speech, physical, occupational or respiratory therapies they have scheduled each day.
- Prepare patients and their families for future self-management and decision-making responsibilities by fostering patients' independence and goal achievement.
- Identify comprehensive care plans based on patient goals and maximum potential.
- Participate in the interdisciplinary team process at team conferences and offer input into team decision making.
- Current Registered Nurse (RN) license that allows you to practice in our state
- Graduate of an accredited nursing program
- Basic Life Support (BLS) from the American Heart Association required upon hire
