- Develop a person-centered treatment plan that focuses on addressing the needs of the client to include family engagement when applicable
- Coordinate care for the client with family and other involved individuals, i.e. school personnel, probation officer, mentor, DHHS, medical personnel, etc
- Provide individual, family therapy, and case management, as needed to each client/family on his/her caseload
- Market and network with community partners and current/potential referral sources. Examples of this include but are not limited to attending community collaborative meetings, distributing marketing material, providing a description of services provided
- Complete all service plans, progress notes, discharge summaries and all other necessary documentation in a timely manner
- Represent Samaritas mission and vision with community partners. Exercise excellence and integrity as the job requirements are fulfilled
- Monitor the quality of overall treatment services to clients on his/her caseload; and evaluate the adequacy of these services to meet treatment objectives
- Participates in regularly scheduled staff meetings, therapist meetings, and any others as deemed necessary by the SUD Leadership
- Represent Samaritas mission and vision with community partners. Exercise excellence and integrity as the job requirements are fulfilled
- Responsible for timely Quality Assurance compliance as related to employee’s roles and functions
- Possess a working knowledge of licensing and accreditation standards and work to assure compliance with such standards
- Utilize educational opportunities and resources offered by the agency and through the profession for continued development of skills and clinical expertise, (conferences, trainings, peer supervision)
- Master Degree in Social Work, Counseling or a related field, from an accredited institution
- Two years of clinical experience working with SUD clientele preferred
- Prior experience working with an interdisciplinary team preferred.
- Minimum one year SUD counseling experience preferred
- Minimum experience working on a team preferred
- Familiarity with SUD documentation preferred
- Valid unrestricted driver's license with good driving record and proof of automobile insurance.
