💼 What You’ll DoAs a Physical Therapist in our Home Health division, you’ll:
  • Coordinate and deliver medically prescribed physical therapy services with a focus on quality, continuity, and cost-effectiveness.
  • Conduct comprehensive evaluations and develop personalized care plans based on musculoskeletal, neuromuscular, cardiopulmonary, and integumentary assessments.
  • Monitor patient progress, adjust treatment plans, and communicate updates to physicians and leadership.
  • Advocate for patients by managing their medical conditions, medications, and functional goals.
  • Supervise Physical Therapist Assistants (PTAs) and collaborate with Home Health Aides to maximize patient outcomes.
  • Educate patients and caregivers on treatment techniques and adaptive equipment.
  • Participate in chart audits, quality improvement initiatives, and discharge planning.
  • Mentor new employees and serve as a clinical instructor for students.
🧾 What You’ll Need
  • Licensure: Physical Therapist license (Wisconsin)
  • Certifications: BLS for Healthcare Providers (AHA) – must be obtained within 6 months
  • Education: Bachelor’s Degree in Physical Therapy
  • Experience: Minimum 3 years in acute hospital, home health, or rehab setting
  • Skills: Strong clinical judgment, communication, organization, and decision-making abilities
  • Tech: Basic proficiency in Microsoft Office (Word, Excel, PowerPoint)
  • Transportation: Valid driver’s license and insured, functional vehicle
