- Actively participates in creating and implementing improvements.
- Supports the nursing process to achieve outcome focused patient care by following the plan of care in order to achieve identified patient outcomes.
- Observes and collects data to contribute to the assessments of the patient and family in order to develop an outcome focused plan of care.
- Follows the plan of care in order to achieve identified patient outcomes.
- Intervenes in emergency situations as appropriate within scope of practice.
- Communicates effectively with patient/family, physicians, and the interdependent team regarding the care of the patient.
- Contributes to the achievement of patient and unit outcomes (Safety, cost, quality, satisfaction, engagement) by working collaboratively with inter and intradepartmental teams.
- Utilizes the nursing process to provide holistic, compassionate, safe, high quality, population-based care
- Observes and collects pertinent data and/or clinical findings to enhance patient outcomes.
- Provides patient educational information appropriate to the developmental level, learning needs, readiness, cultural values, and beliefs of patients and families.
- Demonstrates practices that support safety with recognition of variations in populations served, preventing harm to patients, families, employees, and themselves.
- Demonstrates behavior consistent with the standards, scope of practice, ethics, and characteristics of a licensed professional.
- Basic ability to use a computer and applications that are associated with performing basic work tasks (navigate in Windows, Outlook, epic EHR, etc.).
- Skilled in developing effective rapport with customers, co- workers, or families, actively listening to develop a positive connection.
- Ability to interact effectively, listen actively and develop positive working relationships with diverse customers and staff honoring different world views.
- Practical Nursing diploma from an accredited school of nursing
- 1-3 years experience
- Trade or Vocational Certification
- Basic Life Support (American Heart Assoc or Red Cross)
- MN Licensed Practical Nurse (LPN)
