Rapid response means that the patient will get to the most appropriate level of care faster and expedite their journey to wellness.Visit us online atBehavioral Health Connections (BHC) in Tempe, AZ is seeking a dynamic and talented LPN Referral Management Full Time Nights - Varied 12 hours Shifts that will review assessment referrals and work closely with the RN and other clinicians, to determine the appropriate placement of patients, seeking treatment. Assist the call center specialist with other duties, to enhance referral managment process.Job Duties/Responsibilities:
  • Reveiw assessment referrals received and collaborate with RN to assess patient needs.
  • Assist with coordination of patient care
  • Maintain patient recoreds, updating databases, and ensuring accurate documentation of patient information
