Requirements include:

A current Registered Nursing license for the State of Rhode Island

Prior clinical experience, preferably in home care with a working knowledge of OASIS

Knowledge of Medicare and Medicaid guidelines a plus

The ability to work independently to both assess and treat clients in home settings

To educate the client, their families and their designated loved ones

To communicate effectively with physicians, case managers and other professionals to coordinate care and offer reports as required

Basic computer skills

Current vaccinations as required by governmental agencies

Other documents as required

Responsibilities include:

Perform tasks as assigned

Developing patient plan of care

Supervising certified nursing assistant

Managing patient care and communicating with physicians
