Accept, review, and follow up on referrals from law enforcement agencies received on the 800 lines for clients in need of SUD services
Conduct “warm hand off” by escorting clients to designated treatment locations or arranging their transportation.
Supervise case managers to ensure clients are engaged with the treatment team regarding patient attendance, participation, and any barriers or needs to support treatment involvement.
Distribute Harm Reduction Materials: Deliver harm reduction supplies and resources to individuals within specified zip codes, including syringes, condoms, naloxone, and educational materials.
Community Engagement: Establish rapport and trust with community members while providing information on harm reduction strategies, substance use disorder treatment options, and access to support services.
Outreach and Education: Conduct outreach activities such as street-based outreach, community events, and educational workshops to raise awareness about harm reduction principles and promote safer practices.
Collaborate with Partners: Collaborate with local organizations, healthcare providers, law enforcement, and community leaders to coordinate outreach efforts and facilitate access to resources for individuals in need.
