- Keeping the HumanGood Memory Support Program Philosophy at the core of all planning and implementation of activities and care to meet the specific needs of residents
- Collaborating with Clinical partners to coordinate resident moves and transfers within the Memory Support department
- Assure that resident profile is completed (prior to move-in preferable; no later than 3 business days after move-in) and shared with team members;
- Conducting orientation and promoting support and/or educational groups for residents and family members
- Coordinates with Director of Wellness and Assisted Living when significant changes of residents occur which may lead to level of care change or adjustment;
- Preparing and updating Level of Care Transfer Notices and Service Plans for Memory Support residents in collaboration with the Director of Wellness and Assisted Living
- Maintaining readiness for licensing and accreditation surveys
- Completing required state training programs for Memory Support
- Performing other critical duties and responsibilities as required
- 1 - 2 years related experienced related to dementia care'
- Minimum 1 year experience supervising team members for success, direction, and meeting resident needs
- prior related experience or any equivalent combination of training and experience which provides the required skills, knowledge and abilities.
