Our collaborative model supports over 4,500 patients living in senior communities across Minnesota—and we’re still growing.Position Summary:We are looking for a Physician Assistant (PA-C) who enjoys working independently while being backed by a supportive clinical team. This is a mission-driven, patient-first opportunity to truly connect with your patients and influence their health outcomes.Responsibilities:
Conduct primary care visits in assisted living and memory care settings
Develop care plans, manage medications, and complete orders
Serve as a key communicator between providers, patients, and staff
Document encounters and assessments using EMR systems
Qualifications:
Certified and licensed as a PA in Minnesota
Clinical experience with aging adults strongly preferred
Effective communicator with great follow-through
Valid driver’s license and reliable transportation
