Overview: The Caregiver Advisor (CGA) serves as a primary resource for both those in the community who act as caregivers for someone 60+ (or an adult 18+ with a disability) and for those who are 55+ who are actively raising a grandchild. The Caregiver Advisor stays in close contact with all those enrolled in the program. As needs of caregivers and grandparents in the program arise, the Caregiver Advisor acts as a supportive ear and trusted advisor who matches participants with community resources. The Caregiver advisor monitors participants for signs of caregiver stress and offers them coping strategies. The role of the full-time Caregiver Advisor is also one of research: The CGA is always looking for new information, tips, time savers, and opportunities within the community to aid caregivers and grandparents raising grandchildren in their roles. Responsibilities: Contact each client in the program at least monthly. This can be on the phone or in person. In person contact can be at CRIS by appointment or a home visit. During the call or visit, determine needs a/o concerns of client. Using Options Counseling, assess needs of each client and meet relevant SIS-related needs, including completing applications for Benefit Access, HDM referrals, and Medicare and Medicaid Enrollment. Refer Clients to appropriate services and/or agencies within Champaign County to meet their needs. Agencies may include but are not limited to: Health Department, Home Health Providers, LTC facilities, day care centers, Land of Lincoln Legal Services, etc. Advocate on behalf of the client providing support and services based on the client’s preferences as outlined in their Care Plan and/or documented goals. Research resources, tips, and coping strategies for caregivers; participate in writing and dissemination of monthly CG/GRG newsletter. Complete appropriate case notes, care plans, action plans, and documentation forms on all Caregiver clients. Provide assistance to Caregiver clients in the completion of forms for Social Security, Medicare/Medicaid, Circuit Breaker, Energy Assistance, etc. Provide options counseling as appropriate to help client determine which course of action to take in futures planning, financial obligations, housing, etc. Solicit feedback through surveys on how clients are receiving the program and the satisfaction of the needs being met. Conduct a caregiver/grandparent support group that meets at least monthly. As requested by supervisor or Executive Director, conduct evidence-based classes, including but not limited to: Chronic Disease Self-Management Program, Stressbusters for Caregivers of persons with dementia, Fit & Strong and Aging Mastery. Facilitate community events to educate, inform and assist individuals to make informed decisions and assist with Medicare, Medicaid, LIS, Benefit access, Social Security, Transportation and any unmet needs that are in the community as a whole.
