- Minimum of one year experience in long-term care.
- Basic knowledge of dementia and related care needs of the residents.
- Ability to model good care practices and resident communication strategies.
- Strong verbal communication skills, including ability to give direction in a professional and pleasant manner.
- CNA certification in good standing preferred.
- Has completed Meaningful Moments training.
- Assist in orientating and mentoring new caregivers.
- Acts as a role model for all employees.
- Supports and upholds all company policies and care philosophy.
- Acts as a leader and a voice for caregivers, when needed.
- May attend and participate in family care conferences.
- Answers questions and provides help and support to all caregivers.
- May assist in ordering wellness supplies, at the direction of the Health Services Director.
- Assists with supervision and scheduling of caregivers, provides information regarding caregiver performance issues, to the Charge Nurse, or in the absence of the Charge Nurse, to the Health Services Director.
- Provides necessary and pertinent resident information to shift Caregivers, in conjunction with the Charge Nurse and/or Health Services Director.
- In conjunction with the Charge Nurse, receives report from other Lead Caregivers and gives report to on-coming Lead Caregivers.
- Is assigned other tasks at the discretion of the Health Services Director and Administrator.
- Provides Meaningful Moments mentoring to ensure each caregiver consistently demonstrates the skills to provide Meaningful Moments for the residents and their families.
- Assists with resident medication assistance/administration, in coordination with the Charge nurse, where allowed by state regulations.
- May assist the Health Services Director in the development and implementation of resident service plans.
- Provides personal care assistance to residents as needed, including bathing, dressing, grooming, eating, toileting, changing bed linens, and providing ambulation assistance.
- Reports observations and changes in resident condition to the Charge Nurse, and/or the Health Services Director.
- Provides appropriate resident care documentation, in accordance with JEA policies and procedures.
- Assists with resident care monitoring systems such as weights, skin, and vital signs, at the direction of the Health Services Director.
- Leads and/or assists with scheduled or spontaneous resident activities to enrich the daily lives of the residents, in coordination with the Program Coordinator and/or Health Services Director.
- Able to work full-time, as scheduled on various shifts.
- Medium work - Ability to exert 20-50 pounds of force occasionally, and/or 10-20 pounds of force frequently, and/or up to 10 pounds of force constantly to lift, carry, push, pull or otherwise move objects.
- Must be able to see well enough to write and read, and to visually interact with residents, families, and staff.
- Must be able to speak and hear well enough to verbally interact with residents, families and staff.
