- • Coordinates care for all services being provided after the ACM/discharge planner provides the patient with freedom of choice for a home services agency and contacts the HS Liaison with an order for Home Services.
- Anticipates supply and equipment needs required to provide home services.
- Meets with the patient to explain home services being provided and obtains necessary documentation to provide care.
- Assesses that services ordered are appropriate.
- Documents in the hospital record the plan of care for discharge with the name of the agency, phone number, start of care date and services ordered.
- Accepts out of area referrals and patient requests for another agency.
- Gives referral to Access Services staff to arrange for services.
- Enters patient data and appropriate documentation into the appropriate Home Services databases.
- Coordinates with HS scheduling team to determine start of care date for home services.
- Works with HS office-based Access and Insurance staff, and Infusion Coordinators and Pharmacists to facilitate a smooth discharge for patients.
- Works with the Marketing and Sales Manager, and HS Department Director to update hospital staff on HS programs and contracts.
- Maintains productivity requirements in facilitating timely referrals to Home Services.
- Keeps current regarding community health issues and regulations.
- Communicates with others in a warm, helpful, and professional manner while simultaneously building credibility and rapport.
- Gives high priority to customer satisfaction and anticipates customer needs.
- Preferred - Bachelor's Degree Nursing.
- Preferred - 1 year Full-time experience in home health or hospice.
- Preferred - 1 year Full-time experience in home infusion.
- Preferred - 1 year Full-time experience in discharge planning or care management.
- Preferred - Experience in home care and in positions that required extensive customer service skills.
- Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
- Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
- Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
- Other Qualifications: An established record as a strong team player working closely with others.
- Computer Skills: Ability to operate a PC and learn new programs.
- Other Skills and Abilities: Ability to teach and initiate infusion/enteral therapies and teach health care procedures to patients/family members.
- Other Skills and Abilities: Excellent listening skills, ability to be flexible and self-motivate.
- Required upon hire: In compliance with the Providence Health and Services-OR Human Resources policy #302 Background Checks, a criminal background check is required for this position, based on assigned duties.
- Required upon hire: National Provider BLS - American Heart Association (Vendor Managed)
- OR: Required upon hire: Oregon Registered Nurse License (Vendor Managed)
