This position is distinguished by its responsibility for the overall administration and supervision of the transportation and appointments for Skilled Nursing residents.What You Will Do:
  • Responsible for planning, organizing, and overseeing appointment and transportation for all Skilled Nursing residents.
  • Assists with the formation of the Restorative care piece for Skilled Nursing residents. This position will work with CNAs on training with CNA’s and assisting CNA’s that are transitioning out of physical therapy. This will include informing staff of the residents’ changes during their physical therapy while in Skilled Nursing.
  • Assists with all CNA task as needed including helping with resident feeding, showers, activities, any other tasks need to assist CNAs.
  • Maintain weekly Elopement binder in compliance with Federal and State laws and regulations; ensures that Elopement binder meet standards for audits.
  • Conducts room rounds with other staff to make sure all residents needs are met and assist with any tasks that other CNAs might need help with.
  • Comply with applicable federal, state, and local laws, rules, and regulations; maintains knowledge of and follows all company policies and procedures.
  • Assigns and updates CNA daily shower sheets for residents.
  • Promotes and participates in continuing education courses for staff, team member, resident, and families.
  • Make reports and recommendations to the Director of Nursing as necessary or required concerning the operation of the department.
  • Assigns and updates CNAs daily assignments sheets and staffing sheets. staff work schedules and assignments as changes occur.
  • Maintains compliance with all personnel policies, established community policies and procedures.
  • Monitor, purchase, stock and maintain all department supplies within budget.
  • Attend in-service classes and staff meetings, as required.
  • Dependable and able to meet schedule requirements (attendance and punctuality).
  • Other duties as assigned by Supervisor.
