Job Category: Social ServicesJob Description:tlantic Health System is seeking a Per Diem Social Worker for Case Management at Chilton Medical Center.Essential Function/Responsibilities:
  • Provide Social Work coverage to facilitate discharges, meet with patients and families to assess and provide discharge planning support, resources, referrals, transfers to Extended Care and inpatient psychiatric facilities.
  • Work closely with the Healthcare Team to implement the plan of care, obtain insurance authorizations as required, communicate with community resources and area facilities.
  • Phone, Fax and Electronic documentation a must.
Qualifications:Minimum Education:
  • MSW and LSW
Minimum Experience:
  • 1-year medical setting (acute care hospital) or 1-year externship would be consider preferred
