- Interprets and reviews medication orders for correct drug, dosage, route, frequency, allergies, therapeutic duplication, interactions, contraindications, and potential adverse events for all ages served
- Ensures drugs are available to patients at the time they are needed to avoid delays in therapy and promote positive patient outcome
- Performs pharmaceutical calculations
- Prioritizes workflow by patient impact
- Supervises/performs production of compounded products (sterile and nonsterile, hazardous and nonhazardous) for first doses, scheduled deliveries and batched products
- Provides technician oversight and assumes role of supervisor in routine daily activities
- Oversees dispensing and reconciliation of medications including controlled substances; management of expiration dates and drug recalls
- Maintains knowledge of local, state and federal regulations regarding pharmacy practice as well as knowledge of all applicable accrediting bodies medication management standards (e.g. the Joint Commission, HFAP, etc.)
- Demonstrates supervisory skills necessary to plan, organize and direct activities in the pharmacy in a cost effective manner
- Effectively utilizes pharmaceutical references
- Serves as a drug information expert and resource regarding medication management
- Has a working knowledge of pharmacy operations including: pharmacy operating system, use of patient record, purchasing, inventory and formulary management, automation, packaging, USP 797, USP 795, USP 800, and pharmaceutical waste
- Initiates, documents, and participates in department activities that promote positive patient outcomes. These activities may include, but are not limited to:
- Identifying medication process issues and implementing solutions
- Clinical and cost savings initiatives, therapeutic drug monitoring and appropriate formulary utilization
- Provides comprehensive pharmacy clinical services including but not limited to: anticoagulation management/monitoring, pharmacokinetic dosing, renal dosing, nutritional support services
- Participates in antimicrobial stewardship and opioid stewardship
- Participation in Code Blue activities as applicable to site
- Medication usage evaluations
- Adverse event reporting
- Working with inter-professional teams to optimize order sets, automated dispensing cabinet inventory, and medication related policies
- Assists in the orientation and training of technicians and students
- Serves as an active member of the inter-professional team on patient care units. Participates in unit based huddles and activities if available. Knowledgeable in unit’s metrics, goals, and works with staff on unit to help achieve goals.
- Assists in data collection as assigned
- Promotes formulary therapies to the medical staff, contacting providers with alternatives and using knowledge of medication formulary, current interchanges and non-formulary medication process to make appropriate recommendations
- Provides medication counseling to patients
- Performs medication reconciliation for patients
- Works with inter-professional patient care teams to help patients with transitions to care, including but not limited to helping prepare patient for discharge
- Acts as preceptor for students as applicable
- Communicates with Residency Program Director when needed and in appropriate time frame
- Develops a project to be presented at a pharmacy conference
- Responsible for documented completion of residency program completion requirements as outlined in the training agreement
- Graduate of or candidate for graduation from an ACPE-accredited Doctor of Pharmacy program (or one in the process of pursuing accreditation) or has a Foreign Pharmacy Graduate Examination Committee (FPGEC) certificate from the NABP
- Eligible for licensure as a pharmacist in the commonwealth of Kentucky by the start of the residency program
- Kentucky Board of Pharmacy intern license at the start of the residency program
- Kentucky Board of Pharmacy pharmacist license within 90 days of the residency start date
