  • Work directly with supervising Registered Nurse or Therapist.
  • Personal Care such as bathing, dressing, grooming, skin care, and toileting for patients in their home.
  • Follow HHA Care Plan set forth by supervising nurse.
  • Ability to document visits electronically and in a timely manner per agency policies.
  • Communicate any changes with the patient to the Nurse or Physical Therapist.
  • Responsibilities may vary based on patient's needs.
  • Be an advocate for your patients and their families as you provide care to allow them to maintain a safe quality of life.
HHA Qualifications:
  • Licensed as a Certified Nursing Assistant in the state of Illinois in good standings
  • Minimum 1 year of home health, caregiving experience, hospital, SNF, or ALF experience.
  • Valid Driver's License and reliable vehicle with current insurance.
  • Demonstrated ability to work as a team with supervising RN and interdisciplinary team.
  • Strong communication and interpersonal skills.
  • Comfortable with technology and must have a smart phone or mobile device.
  • Current CPR certification.
