You’ll play a key role in reducing hospital readmissions, managing chronic and acute conditions, and guiding patients through transitional care with compassion and continuity.Core Responsibilities:Provide in-person medical care to patients in assigned post-acute facilitiesPerform assessments, manage treatment plans, and update documentation in the EMRLead or collaborate with facility teams to ensure quality outcomes and smooth transitions of careParticipate in Trumen’s value-based care and transitional care programsDevelop strong working relationships with facility staff and leadershipServe as a liaison between Trumen and facility teamsBe available in facilities during regular business hours (Monday–Friday)Qualifications:MD or DO with a current, unrestricted Texas medical licenseBoard Certified in Internal Medicine, Family Medicine, or Geriatric Medicine1+ year of experience in post-acute, geriatric, or SNF care preferredComfortable practicing independently in skilled nursing facilitiesStrong clinical judgment, communication skills, and a patient-centered mindsetReliable transportation within the San Antonio areaAbout Trumen Physicians + Associates:Founded in 2010, Trumen is a physician-led organization committed to improving outcomes in post-acute and transitional care settings.
