We are currently seeking a full-time RN Care Coordinator to join our team on-site in Somerville, NJ.Position Summary:The RN Care Coordinator plays a vital role in facilitating care for high-risk or chronically ill patients. Working in collaboration with patients, caregivers, providers, and community partners, the Care Coordinator ensures effective care planning, communication, and follow-up across the healthcare continuum.This is a rewarding and dynamic position for an experienced RN who thrives in a collaborative, fast-paced environment and is passionate about improving patient outcomes.Primary Responsibilities:Coordinate care for 10–15 patients per day through a mix of:Face-to-face visitsTelephonic outreachCollaboration with treating providersDocumentation review, analysis, and care planningConduct extended patient engagements (30–45 minutes each) for approximately half the workdayProvide ongoing follow-up via secure email, phone calls, text messages, and other communications with:Patients and caregiversPrimary and specialty care providersCommunity organizations and social servicesServe as a liaison between patients and the healthcare system to support care transitions and reduce barriers to careDocument all encounters, follow-ups, and care plans accurately in the electronic health record (EHR)Assist with clinical duties as needed (e.g., vitals, vaccine administration, PPD readings)Provide front desk support when requiredPerform other duties as assignedQualifications:Required Education & Licensure:Registered Nurse (RN), licensed in the State of New JerseyGraduate of an accredited nursing programEducational background in Nursing, Public Health, Behavioral Science, or related fieldsRequired Experience:5–10 years of clinical experience in one or more of the following settings:HospitalAmbulatory carePost-acute careHealth plan or benefits managementDisease management or care coordinationDesired Skills & Competencies:Highly organized with strong verbal and written communication skillsDemonstrated ability to work independently and as part of a teamStrong decision-making, critical thinking, and problem-solving skillsComfortable engaging with diverse patient populations and interdisciplinary teamsProficient in Microsoft Office (Word, Excel, PowerPoint, Access) and EHR systemsMaintains confidentiality in accordance with HIPAA and other regulatory guidelinesProfessional, reliable, and compassionate
