Growing together.The LPN’s primary role is to support patients with chronic conditions understand and manage their disease.This position follows a hybrid schedule with three in-office days per week.Primary Responsibilities:
  • Deliver exceptional customer service and maintain established quality control standards
  • Help patients understand their assigned care plans and provide outreach to those who are not engaging
  • Redirect patients to Primary Care as needed to ensure comprehensive care
  • Stay up to date on current care plans and expansions
  • Maintain accurate patient records and data
  • Support patients in understanding and managing their chronic conditions, promoting better health outcomes

  • Graduation from an accredited school of nursing
  • Active Licensed Practical Nurse license through the Washington State Department of Health
  • Basic Life Support for Healthcare providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross)
Preferred Qualification:
  • 2+ years of experience working as an LPN

The hourly pay for this role will range from $20.00 to $35.72 per hour based on full-time employment.
