As an LPN in Home Health, you will provide direct patient care under the supervision of a Registered Nurse. You'll work one-on-one with patients in their homes, delivering clinical care, educating patients and families, and supporting individualized care plans.Responsibilities:Provide nursing care to patients in their homes as outlined in the care planMonitor patient conditions and report changes to the RN or physicianAdminister medications and treatments as prescribedEducate patients and caregivers on proper care techniquesMaintain accurate and timely documentation of patient careEnsure compliance with all federal, state, and company policiesQualifications:Current and unrestricted LPN license in IndianaAt least 1 year of nursing experience (home health experience preferred)Current CPR certificationReliable transportation and valid driver's licenseStrong communication and time management skillsAbility to work independently in a home settingBenefits:Flexible schedulingWeekly pay and direct depositSupportive team and 24/7 on-call clinical supportMileage reimbursement (if applicable)Opportunities for long-term placements
