Summary:The stroke program coordinator will oversee the development and operation of the regional stroke program. This position works closely with the program manager, other system coordinators, accrediting bodies and other core stroke team members. This RN will assist in obtaining/maintaining hospital stroke certification through data abstraction/input/submission/analysis and continuous quality improvement. This position applies nursing clinical practices and case management to patients who have suffered from stroke Responsibilities:(40%) Collaborates and establishes rapport with providers, clinical specialists, and community agencies. Performs extensive medical record reviews and concurrent chart data abstraction for all stroke patients.(30%) Coordinates data collection, data entry and reporting of data. Submits required data to registry and other regulatory agencies as requested. Prepares daily, monthly, quarterly and specialty reports as requested. Maintains quality dashboard.(20%) Assures department compliance with accreditation standards. Holds/attends department meetings. Works collaboratively with the program manger on process improvement initiatives.(10%) Active member of the Stroke/Case Review committees. Oversees and conducts required meetings as directed by the stroke program medical director/manager.OTHER DUTIES AND RESPONSIBILITIES:Assists in the development and implementation of the stroke educational program to the medical staff working with stroke patients.Provides appropriate and clear communication both oral and written - to all customers.Creates a climate of clinical excellence that results in improved patient care and optimal outcomes.Maintains awareness of current national and accrediting body trends, practice changes and research findings related to the delivery of stroke care and shares this knowledge with clinical leaders.Assumes other duties including: Is accountable for ongoing acquisition of neuroscience nursing knowledge and skills - especially in the area of stroke and applies accordingly. Identifies opportunities and collaboratively works to implement new neuroscience programs and/or clinics based on identified needs presented by physicians, allied health professionals or patients.Other duties as assigned. Certifications and Licensures:BLS - Basic Life SupportRequired Equivalent Experience - Within 1 month of hire or transferNSC - Nursing Specialty CertificationPreferred Equivalent Experience - Specialty Nursing Certification or Advanced Nurse Certification preferred within 4 years of hire or transfer to position as defined by the department leadership.RN - Registered NurseRequired Required Education:Education: Associates DegreeComments: Bachelor’s degree in nursing at time of hire or transfer to position preferred. Associates prepared staff must obtain bachelor’s degree in nursing by 1/1/2025. Other Information:EXPERIENCE, TRAINING AND SKILLS:Knowledge of Neuroscience nursing as normally acquired through 3-5 years of clinical experience working with neurology and/or neurosurgical patients.Knowledge of general nursing principles, practice skills, organizational and regulatory functions required.Demonstrated strong clinical leadership skills including excellent verbal and written communication.Demonstration of effective team interaction and strong problem solving skill required.Excellent customer service relations-commitment to community service education.Proficiency with computer applications including Access, Outlook, Mysis, Excel and Work required.Ability to work under pressure.MyMichigan Health is a technology driven organization and employees need to demonstrate competency in Microsoft Windows. Credentials as a Certified Neuroscience Registered Nurse preferred.PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS:Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position.Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.Overall vision and hearing is necessary with or without assisted device(s).Frequently required to sit/stand/walk for long periods of time. May require frequent postural changes such as stooping, kneeling or crouching.Some exposure to blood borne pathogens and other potentially infectious material. Must follow MyMichigan Health bloodborne pathogen and TB testing as required.Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis.Physical Demand Level: Light.
