Physical TherapistDepartment: Acute Care FLSA Status: Non-Exempt Reports To: CEOJob Summary:As a CCMSD Physical Therapist (PT), you will be responsible for assessing, planning, organizing, and participating in rehabilitative programs that improve patient mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury. You will demonstrate advanced knowledge and practice level competencies. The CCMSD PT will provide exceptional direct patient care with adherence to safety standards and guidelines to ensure optimal patient safety, customer service, and optimum patient outcomes. The CCMSD PT will provide education to patients and their families and collaborate with other healthcare professionals as needed for problem resolution and achievement of identified patient specific goals. You will complete all documentation in a timely manner and adhere to all local, state and federal regulations, codes, policies and procedures to ensure privacy and safety while delivering optimal patient care.Essential Functions:
Patient assessment:
Performs complete and comprehensive evaluation of patient's physical conditions, including mobility, strength, and flexibility, to identify patient's level of function and to determine the treatment plan.
Develop and implement treatment plans:
Develop customized treatment plans based on evaluation findings that include interventions such as therapeutic exercises, manual therapy, and other treatments to enhance functionality and alleviate pain.
Providing therapy sessions:
Deliver individual therapy sessions to patients using evidence-based interventions.
Monitoring patient progress:
Continuously monitoring and evaluating the patient's progress and adjusting the treatment plan as needed.
Patient education:
Educate patients and families on exercises, posture, and body mechanics to aid recovery and prevent injuries.
Collaborating with healthcare team:
Collaborate with other healthcare professionals such as physicians, nurses, social workers, occupational therapists, and speech therapists to ensure a coordinated approach to patient care.
Documentation and record keeping:
Keep detailed, timely, and accurate records of patient evaluations, treatment plans, progress notes, and discharge summaries. Enters timely and accurate charges for services provided as appropriate.
Professional development:
Keep up to date with the latest research, techniques, and interventions in the field of physical therapy through continuing education, professional development, and participation in conferences and workshops.Supervisory Responsibilities:NoneRequired Skills/Abilities:

Graduate of an accredited program in physical therapy (MPT or DPT).
Registered and licensed in the State of Wyoming and be in good standing.
