RVNAhealth is seeking a compassionate and experienced Extended Hours RN Supervisor to lead our Home Health & Hospice clinical operations during evenings, weekends, holidays, and on-call hours. This vital leadership position ensures that high-quality, person-centered care continues seamlessly beyond standard business hours, supporting both patients and clinicians in the field.Key Responsibilities:Clinical Oversight & Patient Care
Triage inbound patient calls and coordinate timely in-home visits
Ensure the accuracy and quality of clinical documentation in HCHB (Homecare Homebase)
Review and administer patient medications in accordance with physician-ordered care plans
Monitor patient health status, detect changes, and initiate appropriate interventions
Provide education and counseling to patients and families
Maintain timely and compliant EMR documentation, ensuring regulatory and policy adherence
Leadership & Supervision
Lead and supervise Baylor RNs, Infusion RNs, and extended hours clinicians
Support clinical staff performance, ensuring excellence in care delivery and documentation
Deliver feedback and coaching to enhance effectiveness, efficiency, and outcomes
Assist in hiring, onboarding, and professional development of team members
Evaluate performance and enforce accountability aligned with organizational values
Operational & Workflow Management
Oversee hospice referrals and manage clinical workflows during extended hours
Act as the primary liaison between daytime operations and after-hours teams
Ensure staffing levels meet patient care demands across shifts
Conduct quality checks to maintain continuous compliance with CHAP, HIPAA, Medicare CoPs, and CT state regulations
Qualifications:
Registered Nurse (RN) licensed in the State of Connecticut
Bachelor of Science in Nursing (BSN) preferred
Minimum of 2 years of Hospice experience required
Current BLS certification
Proficiency with Electronic Medical Records (EMR) and clinical documentation systems
Strong leadership, critical thinking, and workflow management skills
Knowledgeable of hospice regulations, compliance standards, and best practices
