
Yakima Hospice Night Triage RN - PRN

- Triage after hour phone calls from patients/referral sources/providers in both Home Health and Hospice.
- Determine in the triaging process if the patient needs a home visit and complete necessary nursing visits.
- Able to work independently in home settings; nursing home settings; assisted living settings coordinating care with caregivers and family members.
- Able to use teach back skills and other tools to engage the patient and family in goal setting and self-management.
- Able to support difficult decisions for patients/families such as end of life (Hospice) services.
- Coordinate with YMH House Supervisor to assist as a free float when not supporting Home Health and Hospice triage and visit needs.
- Document services performed per agency policy in the electronic medical record.
- Communicate with the day shift triage and the patient’s case manager about the phone call/visit and follow-up needs.
- Complete phone calls for tuck-in or follow-up for newly admitted patients.
- Licensed in the State of Washington as a Registered Nurse
- Current CPR Certification
- Home Health or Hospice experience preferred
- Current WA state driver’s license and proof of insurance

- You will organize patient care by prioritizing, delegating, delivering and evaluating on an ongoing basis
- You will utilize the knowledge base of nursing education and experience to deliver nursing care using the nursing process according to hospital policies, procedures and protocols
- Collaborate with the nursing team to facilitate overall improvement in quality of care, patient experience and cost reduction
- Foster a learning environment for patients/families, nursing staff and members of the health care team
- Advocate for ethical and holistic care promoting the autonomy, dignity, and health of patients from diverse ethnic and social backgrounds
- Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service or internal candidates hired before January 1, 2021 may be considered in satisfaction of this requirement in lieu of an education agreement
- Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred
- Registered Nurse license (RN) in Washington State or multi-state licensure endorsement (MSL)
- Current BLS Certification required
- ACLS or NALS or PALS or TNCC or NRP certification, as required by facility or department
- Certification in specialty area preferred
- One (1) year of experience in acute care setting preferred
