QUALIFICATIONS: This position functions as part of the Community Support Services team providing staff coverage. Must have good interpersonal and organizational skills in order to communicate effectively with referrals and funding sources, consumers, family members, and team members. A Bachelor's Degree in a Social Services field or a Bachelor's Degree and 2 years Human Services experience. USPRA certification preferred or will acquire certification within 18 months of date of hire. Act 33/34 and FBI Clearances required. Must maintain a current driver's license and vehicle insurance as will be required to transport consumers.RESPONSIBILITIES: * Provides casework services to a caseload of 10 + consumers.
  • Completed needed information in the credible system to open and close a consumer to services.
  • Completes initial assessments, reviews, progress notes and all other contacts as needed in the credible system.
  • Schedules appointment and coordinates follow-up care to meet the needs of the consumer.
  • Completes authorizations and intake in a timely manner to facilitate the billing through Value.
  • Meets with each person on the caseload monthly to discuss and document process toward goals.
  • Documents monthly a summary of progress for each person on the case load in credible.
  • Insures that after each encounter with a consumer be it group or individual, that the consumer's signature is captured on the note in credible.
  • Functions as an effective team member in the development of service plans for the consumers.
  • Services as an effective role model for good work habits, good grooming and appropriate social skills.
  • Networks and maintains positive working relationships with those involved in the consumer's life; such as other Mental Health professionals, employers, family members, and significant others in order to provide effective services.
  • Will facilitate groups in an effective manner by having knowledge of the subject, knowledge of the group dynamic and its relevance to the consumers.
  • To develop and design curriculums for groups that will be offered, to meet the needs of the consumers
  • Maintains a database of the consumers in the programs.
  • Maintains a database of the curriculums that have been developed for groups that have been or will be offered.
  • Develops creative ways to challenge consumers to stay involved, increase work skills, improve work habits, and socialize in a way appropriate to the community.
  • Participates in activities on-site and in the community in a professional manner.
  • Attends weekly staff meeting with supervisor and staff.
  • Will complete weekly schedule in credible the Friday prior to the beginning of the next week.
  • Will adhere to the schedule submitted Friday or will notify supervisor of the changes.
  • Attends relevant trainings, workshops, as assigned to advance skills.
  • Is knowledgeable about the safety procedures on and off site.
  • Maintains USPRA certificate or is actively pursuing certificate.
  • Provides at least 80% of direct service each week.
  • Participates in monthly meetings to discuss cases and strategies that are relevant to the caseload.
  • Physical demands of the job require walking, sitting, standing, hearing, grasping, stooping, finger dexterity for writing notes and data entry. May require driving at times.
  • Maintains and upholds Community Counseling Center's policies, respects confidentiality, and exhibits a professional demeanor at all times while at work.
  • Any violation of policies and procedures/job duties will be subject to disciplinary action.
  • Performs other duties as assigned.
  • Performs the essential functions of this position with reasonable accommodations, if necessary.
  • Participates in CCC CQI plan to ensure quality of service to CCC consumers.
Supervision is received from the Rehabilitation Specialist Supervisor who will assign work based on client need. Employee's work will be reviewed through observation in groups, electronic health record data input and content and timely submission of work involved with the program.0 Total number of subordinates reporting to you.Employee will be assigned 10 or more consumers and be responsible for initial assessments, reviews, progress notes and any other content needed for consumer's electronic health record. Employee will need to be able to facilitate groups in an effective manner and have the knowledge of subjects discussed and be knowledgeable in the group dynamic relevant to the consumers. Employee is expected to be able to network and maintain positive working relationships with consumers. Supervision will be individually and with the team. Work assigned should be organized, timely and in proper format expected of the program.
