The Targeted Case Manager (TCM) is responsible for providing person-centered, goal-driven case management services to individuals with behavioral health, substance use, or developmental disabilities. This role involves coordinating care, advocating for client needs, linking individuals to appropriate services, and supporting them in achieving greater stability and independence in the community. Essential Duties and Responsibilities:
- Conduct comprehensive assessments and develop individualized service plans based on client strengths, needs, and goals.
- Coordinate care and services across multiple systems, including medical, housing, education, legal, employment, and social supports.
- Monitor client progress toward goals and adjust service plans as needed.
- Assist clients with accessing community resources.
- Maintain regular face-to-face contact with clients in homes, communities, and office settings.
- Collaborate with interdisciplinary teams, including clinicians, medical professionals, and other social service agencies.
- Ensure compliance with state, federal, and agency documentation standards and timelines.
- Advocate for client rights and ensure culturally competent and trauma-informed care.
- Attend trainings, supervision, and team meetings as required.
- Education: Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or a related field (required).
- Experience: Prior experience in case management, social services, or behavioral health (preferred).

- Frequent travel to client homes or community locations
- Flexible schedule may be required to meet client needs
- Exposure to individuals in crisis or with complex behavioral issues
- To provide case management services to assigned consumers and adhere to all Pathways policies and procedures.
- Ability to sit, stand, and walk for extended periods
- Able to lift up to 25 lbs occasionally
- Valid driver's license and reliable transportation required
