- Administrative Coordinator reports to the management staff and is under the direct supervision of the Care Manager-RN.
- Complete assigned tasks related to care coordination.
- Identify gaps in care and assist members with gap closure.
- Assemble information concerning patient's clinical background from various hospital portals and EMRs.
- Communicate ADT status of members to the provider via EMR.
- Generate tasks and queues to the assigned Care Manager.
- Assists with data entry, faxing, filing and other clerical tasks.
- Request records from provider's offices as needed.
- As directed and supervised by the Care Manager assist in the attainment of members' goals by:
- Calling the identified member to offer a review of the Disease Management programs, discuss gaps in care, and assist with scheduling indicated medical screenings as appropriate.
- Assisting with incoming and outgoing calls, answering questions regarding gaps in care and obtaining information as requested by licensed staff.
- Informing the member how to access resources.
- Arrange provider follow-up appointments.
- Identify HEDIS/STAR measures that have not been completed during the performance period.
- Navigate various practice EMRs and retrieve medical records supportive of HEDIS/STAR measures.
- Submit documentation via payer portals or fax.
- Complies with Customer Service expectations as applicable to the Department and Health Plan.
- Ensure data submitted to payers meets technical requirements for HEDIS/STAR completion.
- Maintain accurate records of review and submission activities.
- Maintain knowledge of quality measure specifications.
- Comply with all departmental policies and procedures.
- Performs other related duties as assigned.
- High school graduate.
- Computer literacy in Word, Excel.
- Excellent written/verbal communication skills, especially the ability to communicate telephonically in a professional and effective manner.
- Ability to handle multiple demands of diverse workload and prioritize urgent issues
- Self-motivated with the ability to work independently, as well as within a team environment.
- Ability to adapt and quickly learn electronic programs.
- Some college education preferred in the allied health professional field of study.
- Healthcare-related experience is preferred.
- Previous EMR experience preferred.
