- The Community Outreach Worker, Telehealth Services, helps develop, implement, and improve telehealth services, focusing on expanding access in rural areas and ensuring an individual-centered approach that reflects client voice and choice.
- It is important to note that this position is hybrid and determined by the needs of the program.
- Administer social determinants of health (SDOH) screening, intake forms, and assessments.
- Assist clients with prioritizing goals and creating client-centered care plans.
- Coordinate with community organizations and resources to help clients meet their needs.
- Provide resources to clients to improve their health literacy and self-sufficiency.
- Take a proactive approach to assist with assigned cases (e.g. help schedule appointments, complete applications, make reminder calls, etc.)
- Maintain client privacy and always uphold confidentiality.
- Demonstrate commitment in adhering to the Path Assist program's prescribed model.
- Participate in weekly team meetings, workshops, and training to expand knowledge of department priorities, while remaining current on new developments, as required.
- Commute to and from client homes and transport clients for appointments, as approved.
- Assist with creating infrastructure for a successful telehealth program in areas of need.
- Assist with developing and planning improvements to the program, correcting identified deficiencies, and enhancing programs and services.
- Assist with developing collaborative and positive relationships with local partners in targeted rural areas to increase communication and trust, including but not limited to churches, civic groups, medical clinics, rural hospitals, other non-profit organizations, etc.
- Assist with developing and maintaining policies, procedures, program plans, and training materials related to telehealth services.
- Develop expert knowledge and skills of online telehealth platform.
- Assure alignment of telehealth services with in-person services.
- Develop and conduct classes and independent training sessions both on-site and remotely utilizing pre-established guidelines and curriculum for end-users.
- Prepare for all users a comprehensive, clear, and understandable set of instructions describing system processes and user support processes.
- Provide and arrange basic technical support and/or general system maintenance.
- Facilitate and coordinate multi-disciplinary team-based care with agency programs, community partners, and medical health homes.
- Track program and training outcome statistics related to training and education.
- Other duties as assigned.
- Strong computer and technology-related skills are required.
- Ability to develop curriculum and deliver informative and persuasive presentations.
- Ability to work independently to plan and coordinate training classes and curricula.
- Knowledge of program development and building relationships in the service community

- Degree in human services, social work, or related field needed. Bachelor's degree preferred.
- Demonstrated behavioral health experience, including treatment modalities.
- Must be trained in Mental Health First Aid or able to complete training.
- Previous experience with using telehealth platforms.
- Previous experience in providing training and education.
- Valid driver's license, proof of liability insurance and ability to drive for agency business.
- Hybrid work setting (in office, in community, and virtual/remote based on program need).
- Laptop and company issued cell phone.
- Travel to rural areas to meet program needs.
- Must be able to work a flexible work schedule including evenings and weekends to meet clients' or community partner's needs as needed.
