They provide both individual and group psychoeducational sessions to support patients’ transitions into the community. The role includes facilitating benefit applications and coordinating medication or residential needs post-release. Discharge Planners maintain contact with community providers and document all activities in health records. The position requires close collaboration with facility and parole staff to ensure continuity of care.Key ResponsibilitiesCoordinate discharge planning services, including MAT, medical, and mental health programsProvide individual and group psychoeducational sessions for patients scheduled for releaseFacilitate benefit applications and connect patients to community resourcesArrange post-release appointments, referrals, and residential placementsDocument all services in the health record and maintain communication with community agenciesAdditional DetailsFull-Time Day Shift!
