If so, this might be the perfect opportunity for you.At Lifetime Assistance, Inc., we're looking for a Transition and Support Specialist to guide individuals and their families through important vocational transitions. From school-to-work planning to hands-on support in job training settings, you'll play a key role in helping people reach their goals and discover what's possible.What You'll Do:Partner with schools (including BOCES), ACCES-VR, and other providers to coordinate thoughtful transition plansOrganize tours, meetings, and assessments to help individuals explore future training and employment optionsSupport students and adults in building workplace readiness skillsMatch individuals to employment or volunteer opportunities and monitor their successMaintain contact with families, offering advocacy and support throughout the processConduct intakes, manage program waitlists and capacity, and document eligibility and progressTrack goals and progress in alignment with agency and state regulations (ACCES-VR, OPWDD, DOL, Medicaid)Help build new connections with community employers and promote available servicesProvide compassionate, hands-on support when challenges arise-whether it's job coaching, social skills training, or crisis preventionWho We're Looking For:Bachelor's degree in vocational rehabilitation, special education, psychology, social work, or a related fieldAt least 1 year of experience supporting people with intellectual or developmental disabilitiesStrong communication and organization skills-you're someone who can juggle multiple partnerships and keep plans movingExperience with transition planning, vocational assessments, or employment support (preferred)A flexible, solutions-focused mindset and a team playerA valid driver's license and access to a reliable, insured vehicleWhy Join Lifetime Assistance?Be part of a mission-driven team helping people achieve independence and purposeGain valuable experience in transition services, community partnerships, and vocational planningEnjoy a collaborative and supportive environment with ongoing trainingMake a real difference-every dayInterested?
