Job Responsibilities*Please note this is a grant-funded position* The HIV Linkage Coordinator supports the planning, implementation, and evaluation of HIV prevention and care initiatives. This role focuses on community engagement, service coordination, data tracking, and program delivery, without direct supervisory responsibilities.
Coordinate HIV prevention and care activities in alignment with local and federal guidelines
Facilitate outreach and education efforts targeting high-risk populations.
Develops long and short term objectives and plans for the HIV Program
Collaborate with community partners, health departments, and service providers.
Assist in grant reporting, data collection, and program evaluation.
Maintain accurate records of program activities and client interactions.
Participate in public health campaigns, events, and training sessions.
Ensure compliance with confidentiality and ethical standards in HIV services.
Develops policies, procedures, protocols, and guidelines for the HIV Program
Assists with budget preparation and monitoring for HIV Program
Assist with marketing and promoting HIV services and interventions to local organizations and stakeholders.
Collect and manage program data; prepare monthly service reports and quarterly narratives.
Minimum QualificationsMaster's degree in a related field from an accredited college or university AND Two years of experience performing work related to area of assignmentORBachelor's degree in a related field from an accredited college or university AND Five years of experience performing work related to area of assignmentOR One year of experience at the lower level Healthcare Prgm Consultant 2 (HCP091) or position equivalent.Note: Some positions may require licensurePreferred Qualifications
Minimum of 3 years of experience in HIV services, public health, or community outreach.
Knowledge of HIV prevention strategies, testing protocols, and linkage to care models.
Strong communication and organizational skills. • Ability to work independently and collaboratively across diverse teams.
Proficiency in Microsoft Office and data management systems.
Valid driver's license and reliable transportation (for outreach activities).
Experience with evidence-based interventions related to Communicable Diseases.
Cultural competency and ability to engage marginalized communities.
Public health experience
