- Follow-up with patients after discharge to ensure continuity of care with resources available in the community.
- Understand that recovery is voluntary and Peer Specialist/Advocate services are chosen, not required.
- Inform patients of available programs and services, provide education and assist in referrals to other community partners that will assist the patient in meeting their goals of recovery.
- Use a strengths-based approach with patients to connect in an open, unbiased way to create space for conversation and openings for recovery. Use first person language to treat individuals with dignity and respect, opening the possibility of honest communication and optimal support.
- Create a partnership-consultant relationship that focuses on collaboration.
- Use unique perspective to aid in the accomplishment of patient goals.
- Assist the patient in developing their recovery plan and help the patient understand their diagnosis.
- Provide practical assistance, mentoring, advocacy, coordination, one-to-one support, problem-solving, and direct assistance to patients to obtain the necessities of daily living and ensure quality of life.
- Excellent verbal and interpersonal communication skills
- Proficient with MS Office, Outlook and Internet Explorer
- Demonstrate an independent work initiative and sound judgment
- Ability to handle multiple tasks simultaneously
- Ability to have a transparent conversation when necessary. Comply with peer specialist advocate code of ethics
- Ability to successfully complete CPI mandated BHS Training and maintain CPI Certification.
- Knowledge of health care field and medical office protocols/procedures
- Maintain professional/appropriate boundaries with patients to maintain partnership-consultant relationship
- Ability to interact respectfully with diverse cultural and socio-economic populations
- High school diploma (or Equivalent Certificate from an accredited program) or higher degree
- 6 Months: Current Internationally Certified Peer Recovery (ICPR) Certification from the International Certification & Reciprocity Consortium (IC&RC) as attested by Recover Wyoming within six (6) months of start date
- 1-2 years of experience working with individuals experiencing behavioral health issues
