- RN licensure for the state(s) you work in
- Clean driving record, and valid driver’s license and auto insurance
- You will review and evaluate each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discuss and verify impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; is available at all times during operating hours to assist clinicians as appropriate.
- You will attend case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement, and conduct quarterly record reviews and communicate findings and recommendations to Clinical Director and hospice personnel.
- You will assist in screening and interviewing, and assist Director and other supervisory hospice personnel in orientation of new employees.
