- Provide ongoing monitoring of physical, functional, psychological, social, and spiritual and safety needs of the Patient/Family/PCG
- Provide skilled, competent nursing care under the direction of the Primary RN to crisis care team patients and their families.
- Receive and implements orders under the direction of the RN from the physician.
- Administer medications and treatments prescribed by the attending or hospice physician in collaboration with Primary RN and/or CTM and documents following Crisis Care Team MAR process and agency policy.
- Provides comfortable, compassionate, dignified and supportive care prior to, during and following death with patient and primary caregiver following Empath Health policies and in collaboration with primary RN/Counselor and other IDT members.
- Provide hospice teams, support, with team visits, during low census and as needed.
- Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year College or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
- Licensure:
o Valid Florida State Driver’s License. * Certifications:o Valid CPR certification from the American Red Cross or American Heart Association
o Current IV Therapy certification preferred. Must obtain the certification within six months of hire date. * For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion.
