
Hospice Team Physician

- Lead Interdisciplinary Group (IDG) meetings and sign off on care plans for approximatly 60–100 patients depending on census.
- Collaborate with hospice referral coordinators on complex patients discharging to home hospice.
- Serve as the collaborating physician for home hospice nurse practitioners.
- Be a resource for symptom management guidance to home hospice nurse practitioners.
- Provide education and support to home hospice staff, including NPs, nurses, and interdisciplinary team members.
- Assist in training and developing a high-functioning hospice team focused on excellence in care and communication.
- Demonstrate a passion for teaching, kindness, and collaborative team-building.
- Sign and prescribe scheduled medication orders for home hospice patients.
- Review and sign Do Not Resuscitate (DNR) orders and other forms for home hospice patients.
- Provide education to home hospice staff as needed on clinical best practices, policies, and symptom management.
- Assist with emergency face-to-face visits when necessary to meet regulatory requirements or urgent patient needs.
- Be available to answer symptom management questions from nurse practitioners and nurses.
- Participate in weekday evening call (Monday–Thursday), approximately 2–3 times per month.
- Provide weekend call coverage approximately 6–8 weekends per year.
- Participate in holiday coverage, typically one holiday per year.
Preferred QualificationsRole PurposeThe physician will provide professional medical services within the practicing Specialty to the best of physician’s ability through direct patient care and spend additional time as necessary to perform other related duties such as completing medical records, providing MyChart consultations and in-basket management, conducting patient-specific education and collaborating with advanced practice providers and care team members.Responsibilities
- Manages the medical care of patient panel by providing or otherwise arranging for inpatient hospital care of physician’s patients, either through regular hospital rounds, making arrangements with one or more hospitalist(s) or other qualified physician to provide coverage for physician’s hospitalized patients consistent with Medical Staff requirements.
- Collaborates with patients, families, and members of the care team to ensure excellent patient care outcomes at the clinic location(s) designated by BJC and any other BJC clinical outreach location to which physician may be assigned as patient care demands.
- Performs and documents medical histories and physicals in the patient’s medical record as required by hospital medical staff bylaws.
- Provides or arranges for call coverage for clinic patients and inpatient call coverage in a manner acceptable to BJC and in accordance with Medical Staff bylaws, while observing and following all BJC policies and procedures and all applicable legal, ethical and professional standards.
- Collaborates and teaches advanced practice providers, support staff or any care team member assigned in the care of physician’s patient panel.
