- The pay for this position is $68.92 per hour
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Schedule: Monday – Friday, 8:00 AM – 5:00 PM
- No weekends, holidays, or on-call required
- Service area includes all of Washington County, MD
- Home Care Home Base (HCHB) experience preferred
- Perform physical therapy evaluations and treatments based on physician orders and referrals
- Complete comprehensive assessments using appropriate evaluation tools and procedures
- Review patient medical history, current diagnosis, precautions, and contraindications
- Establish measurable treatment goals to maximize patient independence and function
- Regularly reassess patient progress and adjust treatment plans accordingly
- Communicate patient status and needs with physicians, patients, families, and care teams
- Consult with medical providers regarding evaluations, progress, and discharge planning
- Recommend and assist in securing adaptive equipment to improve patient function
- Maintain and submit all evaluation, treatment, and billing documentation as required
- Adhere to company policies, HIPAA regulations, compliance programs, and applicable laws
- Supervise Physical Therapist Assistants (PTAs) and aides in patient care activities
- Participate in internal staff development and continuing education to maintain clinical skills
- Current and valid Physical Therapist license for the state of Maryland
- Experience with Home Care Home Base (HCHB) is preferred
- Strong clinical evaluation and treatment skills
- Ability to supervise and mentor PTAs and aides
- Excellent communication and documentation skills
