- Conduct psychiatric assessments, including diagnosis, history, course of illness, and response to treatment, mental status exam.
- Provide ongoing psychiatric care to clients, including evaluation and assessment, diagnosis, and medication monitoring.
- Explain role of NP and medications in mental health treatment and recovery.
- Work in collaboration with the ACT Medical Team, including Psychiatrist and RN.
- Provide crisis assessment and management of clients, both on-site and over the phone.
- May provide wellness activities as needed such as health monitoring, consultation with other healthcare providers, or directing nursing staff to monitor physical health circumstances and conditions.
- Communicate and consult with teammates to shape quality practice.
- Assist the ACT Team Leader/Clinic Director in training staff in knowledge and skills basic to treatment of persons with severe and persistent mental illness.
- NYS licensed/registered NP with specialty area of Psychiatry required
- Must have NPI number
- National Certification through AANP or ANCC
- Master's of Science in Nursing
- Knowledge of evidence-based practices and/or person-centered care a plus
- Must have a valid driver’s license and own transportation
- Circare’s insurance carrier requires that Circare employees who drive personal vehicles for work maintain minimum auto insurance coverage limits of at least $100,000/person; $300,000/accident and that employees provide HR with proof of current minimum insurance coverage (copy of Auto Liability Insurance Declaration Page) on their first day of employment.
