- An experienced professional responsible for directing, supervising, evaluating, and delivering physical therapy services to adult patients at their homes, following physician directives.
- Assess patients using various diagnostic tests and measures to evaluate muscle, nerve, joint, and functional abilities.
- Create a patient assessment upon initial evaluation, including periodic re-evaluations, and develop a plan of care with treatment types and frequencies.
- Suggest adjustments to the care plan to the physician as necessary.
- Maintain up-to-date clinical notes in the medical records on each visit date.
- Treat patients to address impairments and functional limitations, aiming for increased functional independence.
- Educate and instruct patients and their caregivers on the use of medical equipment.
- Educate patients and caregivers on the treatment plan and objectives.
- Attend PT staff meetings and in-service presentations.
- Participate in case conferences.
- Contribute to the development and updates of the physician’s Plan of Treatment.
- Ensure proper safety practices and equipment maintenance.
- Perform any other assigned duties.
- Graduate from an accredited Physical Therapy program.
- Valid Physical Therapy license from NYS Department of Education.
- Minimum one year of experience as a licensed physical therapist.
- Primarily works in non-office settings and may be exposed to outdoor conditions.
- Medium work – Involves exerting up to 50 pounds of force occasionally and/or a negligible amount of fore frequently or constantly to lift, move, or carry objects, including human bodies. Involves both sitting and standing tasks.
